Achieving adequate gylcemic control in the peritoneal dialysis population is very challenging. We have traditionally assessed glycaemic control using HbA1c but its accuracy in dialysis patients remains debated (half-life of hemoglobin and carbamylation may cause assay interference/inaccuracies). Moreover, the type and timing of PD bags may impact blood glucose levels. Thus PD patients may experience greater variations in glucose over a 24 hour period and increased risk of hypoglycaemia that may not be appreciated by a HbA1c evaluation. We introduced Continuous Glucose Monitoring System (GCMS, Dexcom) for routine clinical monitoring of our insulin dependent PD patients. We now report:1)The correlation between HbA1c and average bloods glucose level as measured by the CGMS2)The correlation between the incidence of hypo and hyperglycemia as reported by the CGMS with HbA1c.
